Introduction to the ESR Test

Does your doctor suspect you have some kind of inflammation? Have they suggested you get your ESR Test done? The ESR test helps diagnose whether your body has been suffering from inflammation.

Inflammation is the body’s natural response to injury, disease, infection, or cancer. This is why the ESR test should never be overlooked. It helps doctors decide what kind of treatment one needs to undergo.

The ESR test is also known as the Erythrocyte Sedimentation Test. Erythrocytes are red blood cells filled with hemoglobin. Normally, red blood cells sink slowly. But inflammation makes red blood cells stick together in clumps. These clumps of cells are heavier than single cells, so they sink faster.

If you have been advised to get the ESR test done, you can get yourself tested by Redcliffe Labs. You can book the test online and get the sample collected from your doorstep without the hassle of waiting in long queues to get a blood test done.

What Is an ESR Test?

An ESR test, also known as the Erythrocyte Sedimentation Rate test, helps detect inflammation in the body. It measures how quickly red blood cells settle at the bottom of a test tube within one hour. When inflammation is present, red blood cells may settle faster than normal. Doctors may recommend an ESR test to help detect infections, autoimmune diseases, arthritis, and other inflammatory conditions, or to monitor treatment progress.

What is the purpose of an ESR test?

The ESR test is one of the important blood tests, and it helps doctors proceed with their treatments accordingly and see if the person needs further tests. These are a few important purposes of the ESR Test:

Detect Inflammation:

The primary purpose of the ESR test is to detect inflammation in the body. It is a non-specific test, meaning it does not pinpoint the exact cause or location of the inflammation.

Monitor Diseases:

It is often used to monitor the progress of diseases that cause inflammation, such as rheumatoid arthritis, lupus, and other autoimmune disorders.

Assist in diagnosis:

While it is not diagnostic independently, it can support the diagnosis of certain conditions when combined with other tests and clinical evaluations.

Types of ESR Tests: What's the Difference?

ESR (Erythrocyte Sedimentation Rate) is an important blood test; along with diagnosing inflammation, this test helps to know the levels of certain proteins in the body. These tests are important for getting your ESR test done regularly. Two ways can do the ESR tests:

  • Westergren Method:

The Westergren Method is the most commonly used method for performing the ESR Test. In this method, blood is drawn to the Westergren-Kaltz tube until it reaches the 200-millimeter level. The tube is then kept vertically at room temperature for an hour, and the distance between the top of the blood mixture and the top of the sedimentation of RBCs is measured.

  • Wintrobe Method:

This test is similar to the Westergren method, with the only difference being that the tube used is 100mm. This is the lesser-used method of ESR testing because it is less sensitive than the Westergren method.

Who Should Consider an ESR Test?

You should consider an ESR test if you have symptoms that may indicate inflammation, infection, or an autoimmune condition. Doctors usually recommend this test to understand the possible cause of ongoing symptoms and monitor inflammatory diseases.

  • People with unusual fever: If you have a fever that persists or recurs without a clear cause, an ESR test may help detect inflammation or infection.
  • People with joint pain or stiffness: ESR testing may be advised if you have swollen joints, morning stiffness, or long-term joint pain that may be linked to arthritis or inflammatory disease.
  • People with symptoms of autoimmune disease: Doctors may suggest an ESR test if you have fatigue, skin rashes, muscle pain, swelling, or symptoms associated with conditions such as lupus or rheumatoid arthritis.
  • People with chronic infection symptoms: ESR may help evaluate long-term infections, including tuberculosis or other conditions that cause persistent inflammation inside the body.
  • People with unusual weakness or weight loss: If you feel weak for a long time or lose weight without trying, an ESR test may help guide further medical investigation. Patients already diagnosed with inflammatory conditions.

Doctors may use ESR testing to monitor disease activity and to check whether treatment is reducing inflammation over time.

Symptoms That Lead to the ESR Test?

High ERS levels can be caused for multiple reasons, some of which can be serious. That’s why, along with getting yourself tested, it is important to know the possible reason behind the inflammation causing the rise in the erythrocyte sedimentation rate in the body. These are some of the reasons that cause inflammation and a rise in ESR levels in the body:

  1. Rheumatoid arthritis is a chronic inflammatory disorder affecting joints.
  2. Lupus is an autoimmune disease that can affect various parts of the body.
  3. Polymyalgia rheumatica: an inflammatory disorder causing muscle pain and stiffness.
  4. Vasculitis: inflammation of blood vessels.
  5. Bacterial Infections: Such as tuberculosis, pneumonia, and urinary tract infections.
  6. Viral Infections: Certain viral infections can also raise ESR levels.
  7. Fungal Infections: These can also lead to elevated ESR.
  8. Chronic Kidney Disease: Persistent kidney inflammation or infection can elevate ESR.
  9. Inflammatory Bowel Disease (IBD) includes conditions like Crohn's disease and ulcerative colitis.
  10. Heart Disease: Conditions like endocarditis (infection of the heart valves) can raise ESR.

Cancers:

  1. Lymphoma is a type of cancer affecting the lymphatic system.
  2. Multiple myeloma: a cancer of plasma cells in the bone marrow.
  3. Various Other Cancers: Many cancers can cause elevated ESR due to associated inflammation or infection.

Other medical conditions

  1. Anemia: Certain types of anemia can elevate ESR.
  2. Pregnancy: ESR levels can naturally be higher during pregnancy.
  3. Obesity: Increased body weight can be associated with higher ESR levels.

These are some reasons why your doctor may recommend that you get the ESR blood test done. This test determines whether there is any inflammation in the body, but it does not confirm the specific reason behind it. If inflammation is confirmed, your doctor may ask you to undergo further testing to determine the reason behind it.

What Conditions Can Be Diagnosed With an ESR Test?

Conditions that may be associated with abnormal ESR levels include:

  • Infections: An ESR test may show high levels when the body is fighting bacterial, viral, or fungal infections. Doctors may suggest it when fever, weakness, body pain, or long-term infection symptoms are present.
  • Rheumatoid Arthritis: High ESR levels indicate joint inflammation and rheumatoid arthritis.
  • Autoimmune Diseases: Conditions like lupus, vasculitis, and polymyalgia rheumatica can raise ESR levels. The ESR test helps detect ongoing inflammation caused by an overactive immune response.
  • Tuberculosis (TB): ESR levels are often elevated in chronic infections. Doctors may use it with other tests to assess inflammation and disease severity.
  • Inflammatory Disorders: Diseases such as inflammatory bowel disease and other chronic inflammatory conditions may increase ESR. The test helps doctors understand the level of inflammation in the body.
  • Anemia: Certain types can affect red blood cells, thereby raising ESR levels. Doctors may compare ESR with CBC results to better understand the cause.
  • Kidney Disease: Chronic kidney disease may cause inflammation and abnormal ESR readings. ESR may be used in combination with kidney function tests to improve health assessment.
  • Certain Cancers: Some cancers, such as lymphoma and multiple myeloma, may cause elevated ESR levels. However, ESR alone cannot diagnose cancer and needs further confirmatory tests.

Doctors usually use ESR test results, along with symptoms, physical examination, and other blood tests, to accurately diagnose and plan treatment.

Causes of High ESR Levels:

An abnormal ESR may help with a diagnosis, but it does not prove that you have a certain condition. Other tests are almost always needed.

An increased ESR rate may occur in people with:

  • Anemia
  • Cancers such as lymphoma or multiple myeloma
  • Kidney disease
  • Pregnancy
  • Thyroid disease

The immune system helps protect the body against harmful substances. An autoimmune disorder is when the immune system mistakenly attacks and destroys healthy body tissue. ESR is often higher than normal in people with an autoimmune disorder.

Preparation Before an ESR Test (Fasting & Guidelines)

There are no rules to follow before taking the ESR blood test. However, these are some basic guidelines to be followed:

  • You are not required to fast before taking the double marker test, but if your health requires it, talk to your physician about any special dietary requirements.
  • If you are pregnant, you might be on any medication or supplement. It is important to inform your doctor about that as well. You do not need to panic in such situations; they will also tell you about the solution if they think there is an issue.
  • It is a blood test, so avoiding any workouts or yoga before taking the test is always recommended.
  • It is advised not to smoke or drink alcohol before taking the test because it may alter the test results.

If you follow these basic guidelines before taking the test and keep your physician in the loop, you are ready.

ESR Test Procedure (Step-by-Step Process)

The procedure for the ESR test is:

  • Step 1: The phlebotomist ties a band around your arm to make the veins more visible for blood collection.
  • Step 2: You may be asked to make a fist to make the vein easier to locate.
  • Step 3: The skin area from where the blood will be drawn is cleaned with a disinfectant to reduce the risk of infection.
  • Step 4: A thin, sterile needle is inserted into the vein to collect the required blood sample.
  • Step 5: After the blood is collected, the needle is removed, and a bandage is applied to the puncture site.
  • Step 6: The collected blood is transferred into a vial, properly sealed, and labeled for testing.
  • Step 7: The vial is then sent to the laboratory, where the ESR level is measured and analyzed.

You may feel mild stinging, soreness, or discomfort around the puncture site. If the pain continues or worsens, consult your healthcare provider.

Risks Associated with the ESR Test:

There aren’t any risks directly associated with an ESR Test. However, having your blood drawn may involve minimal risks. Possible complications include:

  • bleeding, from very light to excessive
  • fainting
  • Hematoma
  • Bruising
  • infection
  • inflammation of the vein
  • tenderness
  • lightheadedness

When the needle pricks your skin, you’ll probably feel mild to moderate pain. After the test, you might also feel throbbing at the puncture site.

How to Read ESR Test Results? (High & Low ESR Levels Explained)

Along with getting tested, it is important to understand your blood test reports. This plays an important role in giving you control over your health. You can make major decisions about your treatments with a better understanding of your health reports.

Normal ESR levels:

Men: Typically, a normal ESR is 0–15 mm/hr for men under 50 and 0–20 mm/hr for men over 50.

Women: Typically, a normal ESR is 0–20 mm/hr for women under 50 and 0–30 mm/hr for women over 50.

Children: Normal values can vary but are generally lower than adults.

Keep these normal values in mind, and if you find any abnormalities, you must consult your doctor as soon as possible. Your doctor will look into the reports and provide the treatment accordingly, or they may ask you to get further tests done if necessary.

What Does Abnormal ESR Level Mean?

Abnormal ESR Levels do not identify any disease; they can serve as a warning sign as they diagnose inflammation in the body. If someone has high ESR levels, they consult with their doctors and undergo further testing to determine the issue in their bodies.

There are multiple causes of a high ESR test result. Some common conditions associated with a higher rate include:

  • inflammatory and infectious diseases
  • tissue injury
  • trauma
  • types of lymphoma and multiple myeloma
  • older age
  • pregnancy
  • anemia
  • kidney disease
  • diabetes
  • heart disease
  • blood or vascular diseases
  • vasculitis
  • obesity
  • thyroid disease

So, if your reports show that you have high ESR levels, you must consult with your health provider as soon as possible so they can help you with treatment and suggest all the possible tests you should get after the diagnosis of inflammation.

How do you manage your ESR levels?

Erythrocyte Sedimentation Rate (ESR) is a common blood test that can indicate inflammation in the body. Managing ESR levels involves addressing the underlying causes of inflammation and maintaining a healthy lifestyle. Here are some ways in which you can manage ESR:

  • Follow medical advice:

If your ESR levels are high, work with your healthcare provider to identify and treat the underlying cause. Take prescribed medications for infections, autoimmune diseases, or chronic illnesses as directed.

  • Diet and Nutrition:

Incorporate foods rich in antioxidants and anti-inflammatory properties, such as fruits, vegetables, nuts, seeds, fatty fish, and whole grains. Reduce the intake of processed foods, sugars, and trans fats that can increase inflammation.

  • Regular Exercise:

Stay Active: Regular physical activity reduces inflammation and improves overall health. Aim for at least 150 minutes of moderate aerobic exercise per week, including strength-training exercises at least twice weekly. Remember to maintain a healthy weight, as excess body weight can contribute to inflammation.

  • Hydration:

Drink plenty of water to help your body function optimally and reduce inflammation.

  • Stress Management:

Practice stress-reducing techniques such as meditation, yoga, deep breathing, or other relaxation methods.

  • Adequate Sleep:

Ensure you get 7-9 hours of sleep daily to help your body recover and reduce inflammation.

  • Avoid smoking and limit alcohol.

Smoking increases inflammation and can raise ESR levels, and excessive alcohol consumption can contribute to inflammation as well. So, it is recommended that you quit smoking and drink in moderation.

You may follow these steps, which will help you significantly lower your ESR levels. However, you must always consult with your doctor before making any major lifestyle or dietary changes; they can guide you through the process.

Normal ESR Test Range & Interpretation

Category Normal ESR Range
Men 0–15 mm/hour
Women 0–20 mm/hour
Children 0–10 mm/hour
Older adults May be slightly higher with age
  • Normal ESR usually suggests no major inflammation, infection, or autoimmune activity.
  • High ESR may indicate infection, inflammation, anemia, arthritis, tuberculosis, or autoimmune disorders.
  • Low ESR may occur in some blood disorders, high red blood cell count, liver disease, or abnormal protein conditions.

What Affects ESR Test Results?

Several factors can affect ESR test results because ESR is a general marker of inflammation rather than a disease-specific test.

  • Infections: Bacterial, viral, or long-term infections can raise ESR levels due to inflammation.
  • Inflammatory Diseases: Conditions like rheumatoid arthritis, lupus, vasculitis, and inflammatory bowel disease can raise ESR levels.
  • Anemia: Low hemoglobin or reduced red blood cell count can make the ESR appear elevated.
  • Pregnancy and Menstruation: ESR may naturally increase during pregnancy or around menstruation due to normal body changes.
  • Age and Gender: ESR levels are usually slightly higher in women and may also increase with age.
  • Certain Medicines: Some medicines, including oral contraceptives, steroids, and anti-inflammatory drugs, may affect ESR results.
  • Chronic Diseases: Kidney disease, thyroid disorders, autoimmune conditions, and some cancers may cause abnormal ESR levels.

Doctors usually compare ESR results with symptoms, medical history, and other tests, such as CRP or CBC, to arrive at a clearer diagnosis.

ESR Test vs CRP Test: What's the Difference?

Both ESR and CRP tests help detect inflammation in the body, but they measure different things. An ESR test measures how quickly red blood cells settle in a tube, while a CRP test measures C-reactive protein produced by the liver during inflammation.

  • ESR Test: The ESR test gives a general idea of inflammation in the body. It may rise slowly and may also take longer to return to normal after inflammation improves. Doctors commonly use it to monitor long-term inflammatory conditions, infections, and autoimmune diseases, as well as treatment response.
  • CRP Test: The CRP test detects inflammation more quickly than the ESR test. CRP levels may rise rapidly during infection, injury, or active inflammation and may decrease more rapidly as the condition improves.
