What does LFT measure?

The liver is the second largest organ in the body and plays a crucial role in producing and excreting certain substances.

The components of LFT help to understand each characteristic of liver function. These components are:

Alanine Aminotransferase (ALT): This is a blood test used to determine the level of Alanine Aminotransferase in the blood, which is found in the liver. Alanine is the most used biomarker for liver health. The enzyme was previously known as serum glutamate pyruvate transaminase (SGPT). The ALT enzyme is mainly found in the serum and organ tissues of the liver. Some significant concentrations have also been detected in the kidney, skeletal muscle, heart, and pancreas.

Aspartate Aminotransferase (AST): This blood test is usually performed to evaluate the level of enzyme Aspartate Aminotransferase in the blood which is found in the liver. It is an enzyme which is previously known as serum glutamate oxalate transaminase (SGOT) The AST enzyme is present in all tissues except bone, with the highest levels in the liver and skeletal muscles. The level of AST is increased in the case of trauma, necrosis, infection, and liver injury.

Bilirubin: During the breakdown of red blood cells, bilirubin is formed as a waste product. It is a bile pigment that is brownish-yellow and produced by the liver. It contributes to the colour of solid waste products (faeces.

By the breakdown of RBC about 0.5 to 2 grams of bilirubin is produced daily in the human body. Increment of not processed bilirubin by a damaged liver can cause jaundice, hepatitis, cirrhosis, gallbladder diseases, and hemolytic anemia.

Gamma-glutamyltransferase (GGT): GGT is a very susceptible test for detecting any liver disease. It is an enzyme found in the cell membranes of several tissues, especially in the liver, kidney, and pancreas.

GGT is also found in other tissues including the intestine, spleen, heart, brain, and seminal vesicles. Elevation may occur with diabetes, heart failure, hepatitis, tumors, etc.

Alkaline Phosphatase (ALP): The ALP test is used to measure the amount of Alkaline Phosphatase in the blood. It is an enzyme that helps in the breakdown of proteins. It is primarily produced in the liver and bones, though a small amount is also produced in the intestines and kidneys.

In pregnancy, the placenta also secretes ALP. It transports nutrients and enzymes from the liver, helps develop bones, regulates cell growth, transports fatty acids, calcium, and phosphate. The highest concentration can cause gallbladder stones, cancer, and hepatitis.

Total Serum Protein Total Serum Protein is a combined test for total protein, albumin, globulin, and albumin/globulin ratio (A/G ratio). It measures the total amount of protein in the blood.

The most common protein in the blood is albumin, which prevents fluid from leaking out and transports substances. Albumin also promotes tissue growth and healing.

However, globulin is made up of some alpha, beta, and gamma proteins. Most globulin is made by the immune system, with a small amount made by the liver. Prothrombin time (PT): This parameter aids in the detection and diagnosis of bleeding disorders or excessive clotting disorders. It is a protein produced by the liver. It is one of many factors present in the blood that helps in the clotting of blood suitably.
