What is the Purpose of the Widal S Typhi O Test?

The Widal test is a serological test used to diagnose enteric fever, commonly known as typhoid fever, caused by the bacteria Salmonella enterica serotype Typhi. The test specifically detects the presence of antibodies in a patient's serum against the O (somatic) and H (flagellar) antigens of Salmonella Typhi.

The Widal test's primary purpose is to help diagnose typhoid fever in patients presenting with symptoms such as prolonged fever, headache, gastrointestinal disturbances, and malaise, especially in areas where the disease is endemic. The test measures the levels of antibodies (anti-O and anti-H) in the blood, which typically rise during infection. 

The "O" antigen is a component of the bacterial cell wall and is associated with the polysaccharide structure of bacteria. An elevated titer of anti-O antibodies indicates an active infection or recent exposure, suggesting acute typhoid fever. 

Despite its usefulness, the Widal test has limitations, including false positives due to previous vaccinations, infections with related bacteria, or chronic carriers. Therefore, it is often used in conjunction with other clinical findings and diagnostic tests, such as blood cultures, to confirm the presence of Salmonella Typhi.

Preparations Before the Widal S Typhi O Test:

No special set of rules must be followed before taking the S typhi O blood test. However, these are some basic guidelines to be followed:

  • You are not required to fast before taking the double marker test, but if your health requires it, talk to your physician about any special dietary requirements.
  • If you are pregnant, you might be taking any medication or supplements. It is important to inform your doctor about that as well. You do not need to panic in such situations; if they think there is an issue, they will also tell you about the solution. 
  • It is a blood test, so avoiding any workouts or yoga before taking the test is always recommended.

The procedure for the Widal S Typhi O test:

This S Typhi O test is like any other blood test, and it does not take more than 5-10 minutes for the phlebotomist to take the blood sample. The procedure for getting the test is as follows;

  • The phlebotomist, on arrival, ties a band around your arm.
  • They tell you to make a fist so the veins are properly visible.
  • They disinfect the area from which the blood is to be drawn.
  • Then, a thin, sterile needle is inserted into the vein to draw blood, and after the needle is removed, a bandage is put on the punctured area.
  • After drawing the blood, they put it in a vial, seal it, and label it.
  • This vial is then taken to the laboratory to be tested.

Remember that the punctured area may have a stinging sensation for two days. If the pain lasts longer than two days or is significantly severe, consult a doctor.

Symptoms of Typhoid:

Typhoid fever is caused by the bacteria called Salmonella typhi. The bacteria causes multiple issues in the body, but the symptoms do not occur before 1-3 weeks of getting infected by the bacteria.
