Purpose of the CD4 Percentage & Absolute Count Test

CD4 count, along with an HIV viral load, is used to diagnose HIV infection to -

CD4 cells, also known as T lymphocytes or T-helper cells, are a type of white blood cells. 

The cells help fight infections by identifying, attacking, and eliminating harmful bacteria, fungi, and viruses. The CD4 test measures the number of immune cells present in the blood and is commonly used alongside an HIV viral load test to assess disease progression in people with HIV infection.

CD4 cells are produced in the thymus gland and circulate in the body via the blood and lymphatic system. HIV targets and binds to CD4 cells, allowing the virus to enter and replicate immediately—destroying the cells in the process—or remain dormant for future replication. If HIV is left untreated, the virus continues to multiply, increasing the viral load and reducing the number of CD4 cells. Over time, if the CD4 count falls too low, it weakens immunity and causes AIDS-related symptoms.

Antiretroviral therapy (ART), also called highly active antiretroviral therapy (HAART), can help to control the virus by reducing the viral load and preventing further damage to the immune system. With new HIV symptoms or the start of the new treatment with interferon, corticosteroids, or cancer drugs Must take the test regularly

What do the CD4 Percentage & Absolute Count Test reports indicate?

A CD4 count is reported as a count of cells. Sometimes, results are shown in a percent of total lymphocytes (CD4 percent). Here is what the test results indicate-

  • Normal CD4 Count (500–1,200 cells/mm³): Indicates the immune system is functioning well with no significant impact from HIV infection.
  • Low CD4 Count (below 500 cells/mm³) suggests immune suppression due to HIV progression or other conditions.
  • Severely Low CD4 Count (Below 200 cells/mm³): The immune system becomes too weak to prevent opportunistic infections like Pneumocystis jiroveci pneumonia (PCP) or oral thrush. The CDC classifies people with HIV and CD4 counts below 200 cells/mm³ as having AIDS (stage III HIV infection), regardless of symptoms.
